- Birth Certificate & Cenomar (for Single) (PSA copy)
- Marriage Certificate (if Married) (PSA copy)

- Certificate of Employment (COE) with position, date of hired and salary
- Latest Payslips (last 1–3 months)
- BIR Form 2316 or Income Tax Return (ITR)
- DTI or SEC Registration
